چکیده

The present study describes the clinical and electrophysiological characteristics of sustained bundle branch reentrant ventricular tachycardia treated with electrical ablation of the right bundle branch. Seven patients presented with syncopal episodes, and six of the seven had documented episodes of ventricular tachycardia. All patients had depressed left ventricular ejection fraction with cardiomegaly. Six of the seven had dilated cardiomyopathy in the absence of significant coronary disease. Twelve-lead electrocardiograms in all seven patients during sinus rhythm were remarkably similar; six demonstrated intraventricular conduction defect resembling left bundle branch block, and one showed left anterior fascicular block. All patients showed prolonged His-to-ventricle intervals during sinus rhythm. Sustained ventricular tachycardia (with atrioventricular dissociation) because of bundle branch reentry was induced in all patients during baseline electrophysiological study. The His-to-ventricle intervals during tachycardia were similar to those seen during sinus rhythm. Electrical ablation of the right bundle branch was accomplished in each patient with delivery of two electrical shocks (170-310 J) through electrode catheters. Right bundle branch block developed on their surface electrocardiogram immediately after the ablation. Follow-up electrophysiological studies showed

Bundle branch reentrant ventricular tachycardia after transcatheter aortic valve replacement

Introduction Bundle branch reentrant ventricular tachycardia (BBR-VT) is a well-described entity that accounts for 6% of inducible sustained monomorphic VT during electrophysiology studies. BBR-VT is a macroreentrant VT involving the HisPurkinje system and usually requires some form of conduction delay in the His-Purkinje axis. BBR-VT is commonly seen in patients with nonischemic dilated cardio...

Treatment of ventricular tachycardia-induced cardiomyopathy by transcatheter radiofrequency ablation.

Catheter ablation of ventricular tachycardia was successfully performed in a patient with dilated cardiomyopathy (ejection fraction 38%) and a long history of repetitive palpitations. Holter monitoring showed ventricular tachycardia that had a left bundle branch block QRS configuration with inferior axis deviation and was present for about one third of the daytime hours. At electrophysiological...

Premature ventricular contractions with two QRS morphologies originate from one focus in great cardiac vein

It has been demonstrated that catheter ablation may be a reliable technique for curing ventricular arrhythmias originating from the great cardiac vein (GCV), and there are some challenges and risks with this therapeutic method. Although some reports described successful catheter ablation of ventricular tachycardias originating from the GCV, premature ventricular contractions (PVCs) with left bu...

Successful catheter ablation of focal ventricular tachycardia originating from right bundle branch without making right bundle branch block, using pharmacologic induction by landiolol

Introduction Right ventricular outflow tract tachycardia is one of the most common ventricular tachycardia (VT) observed in young patients, accounting for 60%–80% of all idiopathic VTs. The QRS morphology of this tachycardia is left bundle branch block (LBBB) type, inferior axis, and wide QRS complex. In contrast, focal VT originating from the right bundle branch (RBB) is a rare idiopathic VT. ...
